Can you please clarify the conjoint collaborateur?

I registered a business in France last September and have only just had my paperwork from RSI, the paperwork is entitled Conjoint Collaborator Auto-Entrepreneur. I have tried translating but not sure exactly what it means, but want to make sure I tick the right box. There is first and second option and two boxes underneath. Can you help? I have also still not received any carte vital and tried chasing is this anything to do with that?
