Number of employees, it is just me so is the answer 1 or none?
Number of weeks new establishment was run? Do I put the number of weeks I have worked or the number of weeks I have been registered as auto-entrepreneur?
Number of weeks I expect to work in 2015? I have absolutely no idea. Do I just make an educated guess?
Date started as auto entrepreneur regime? Is this not the same as date de creation de enterprise i.e. the date I have been registered as auto-entrepreneur from.

Section C
Is any of this relevant to me as I do not work from home or rent a work space. I work at institutions e.g. university to teach.

Section D
I have worked in October November and December for just 6 hours per week this year. Next year I will have more than one client. I’m not sure which exoneration is relevant to me, is it just enterprise nouvelles beneficent?

When does this form have to be submitted by?
