completion of 2018 French income tax form

Hi Valerie
I am trying to complete form 2042C. Had no problem in previous years but have a few questions as the form has changed:

title page-Your Revenues. I completed box 5ND as usual. but bottom of that page is note saying- to avoid in 2019 a dual collection….there are boxes populated and I don’t fully understand where the figures come from. Is it the revenue indicated on the tax form avis d’impot? we didnt pay tax last year and box is not populated so we leave as such?
next page has the same thing but boxes populated but have different codes. just ignore?
next page: general info ; we have put our apartment as the operating address in previous years so is this correct this year?
