Do I have to complete and return the CFE 2013 form?

I started as an auto-entrepreneur in 2012 and have just received CFE 2013. Do I have to complete and return this form? There are 2 copies do I return them both? Do I attach the covering letter mention earlier about CFE exemption? Is there an English translation anywhere?
