Do I need insurance responsibility civile professionnel?

I received my institut national de la statistique et des études économiques (INSEE) registration with the système d’identification du répertoire des établissements (SIRET) number.

I am now planning to submit the form Bulletin de déclaration d’activité d’un prestatire de formation. Do I also need to mention on the form the name of my website, or just submit it under my name?
I am planning to start my work from January 2016. Do I need to get the Insurance Responsibility Civile Professionnel starting from now because I have a SIRET number, or can I put start date from January 2016?
