Do I need to complete the AG2R forms?

I read a few answers already but people seem to get different letters and forms from AG2R.

I have received a Certificat d’adhesion.
A form titled Demande d’adhesion Prevoyance Cadre with the major section being Regime de Prevoyance du Personnel Cadre.
A form titled Demande de Renseignements.

Do I need to complete the forms or just let them know I have no employees?
