Help with change of business address?

I need to change my business address here in France. I have printed of this form. I have some questions.

I need to fill out section 1, 6, 9 and 10 only?
At the top it asks my activity. I am a Conseil du Marketing - does this make me a Commerciale, Artisanale or Liberale?
I work from home - in section 6 it asks me Vous excerez votre activite a une adresse professionelle - is the answer yes?

Also I can find no address to send this document to, any ideas?
