How do I terminate my auto entrepreneur business registration online?

I am trying to close my auto entrepreneur business registration in France online, but there are some parts I am unclear about what to enter.

There is a field Denomination du Destinataire - could you please advise what should be entered there?
Under Declarations Sociale the legend says Organisme d’assurance maladie choisi, eg. RAM. But it says please enter first the place of activity - when would RAM be entered or if entered later, what would one enter now e.g the name (or post code) of the place where one works?
After Validation at the end it says ensure you get the certificat electronique - is this provided online after the entry and validation of the data?
