I received a form to fill in and return to RSI. Would you kindly help me to fill it:

  1. They want my social security number but I do not have it yet?
  2. What is Nom the usage?
  3. I’m not married or pacsed, so does it go under Vit Maritalment or should I choose Celibataire?
  4. I want my partner to my carte vitale. I think RSI also wants his Extrait de naissance. My partner does not work at the moment, so should I just leave it blank when they ask profession?
  5. Votre ou vos activite(s) professionnelle(s)?
  6. They ask number siren, I have Siret number, is it the same?
