Letter and form from Direction Generale Des Finances Publiques

I registered as an auto-entrepreneur on 1 October (with your help, thank you) as a Formatrice Anglais (Trainer of English, presentation skills, etc.) I received a letter from the Direction Generale des Finances Publiques a few days ago along with a form/questionnaire to complete and send back within 15 days. The questionnaire asks for:
- My address, telephone number, etc. Don’t they already have that? They sent a letter to me.
- The name of my accountant, but I don’t have one. Am I required to get one now?
- Caracteristiques des locaux effectes a votre activite (Addresse, numero de lot dans la copropriete, precedent occupant, superficie totale, detail de l’afffectation de la superficie a usage , etc) However, I’m simply registered at my home address and I will not carry out my activities (only administration) in my home. I will do all my English teaching and training at the clients’ premises. How should I mention this on the form? If I give them the square footage of my home, it may only create problems. I do not expect to earn more than 32,500 euros per year (I believe that is the correct limit for my type of work), so I do not plan to deduct my office, travel or other expenses.
Perhaps they are asking me for all of this because they noticed that I was registered for the year of 2008 and worked only that year before ending my activities. Then I did the same thing for a year in 2011/12, and now I’ve re-registered again. They never sent me this letter before. Could you please advise me how to respond to this?
