Modifying a declaration of business activity in France?

I want to add soutien scolaire or cours particuliers as my third business activity and I’m filling in the form on lautoentrepreneur.fr. For Section 7, the choices in the drop-down boxes don’t exactly match, but this is what I’ve chosen:

Nouvelle activité exercée: SERVICES DE SOUTIEN A L’ENSEIGNEMENT
Sa Nature: Profession libérale
Son lieu d’exercice: non précisé

Do these look OK? For Section 9, Dénomination du destinataire - is this just my name? I don’t have a general company name, although my main activity is English clubs for children, as a licensee authorised to use the company name.
