Process for closing down auto entrepeneur business?

I have found the forms for closing down the business, but cannot see any specific instructions as to what to do with them. I know that they need to be sent to the relevant CFE office, but should I send all three copies, and should I include proof of identity? Is there anything else I should be sending?
