Providing documents to CPAM
After registering as a micro-entrepreneur, I have received my first letter from l’Assurance Maladie / CPAM asking for documents (passport, visa, birth certificate, bank info, etc) to create my file.
I have three questions about what and where to send the documents:
1 - For “un relevé d’identité bancaire (n° IBAN)”, I am planning to send a print-out of the RIB for my French account. Is this what they need? Also I have two personal accounts and one is dedicated to my micro-entrepreneur activity. Should I send them the RIB for the one dedicated to ME activity OR the one that is not?
2 - For “une copie de votre titre ou de tout document de séjour en cours de validité”, would a scan of the visa page in my passport suffice?
3 - I have been told to send them all the documents (including the letter they sent me) but all I can find for where to send it is a very strange address at the top of their letter (please see below). Could you please tell me what I should put on the envelope I address to them?
