SIE form 751-SD and Wework

I’ve received the Welcome Letter and Memento Fiscal from the tax office, along with Form 751-SD to complete and return. I have a few questions and was hoping you could clarify: 

1. Section 3: *“Vous exercez votre activité à votre domicile et vous n’avez pas d’autre local à votre disposition”* 
  My answer here would be both yes and no, as I primarily work from a WeWork in Paris where I have an annual membership (which offers coworking space but not domiciliation) and occasionally from home. The registered address remains my home since WeWork doesn’t provide domiciliation. 
  As I am registered as an auto-entrepreneur under the profession libérale category (conseil et service marketing), I believe you mentioned that my category is only permitted to operate from home and not via a domiciliation service. Could you confirm how I should complete this section? 

2. Section 4: “Vous êtes membre d’un SCM”*
  I’m unsure if this applies to me. Could you provide guidance? 

3. Section 5: “Autres, ou complément d’informations que vous jugez utile de préciser”
  Is it acceptable to leave this section blank? 

4. Signature Section: “Date, qualité, et signature”
  What should I write for “qualité”? Would “entrepreneur individuel” be appropriate?
