Translation of birth certificate for business registration in France?

You helped me register as an auto-entrepreneur business in France in June 2016. I have not yet received my carte vitale yet but received a letter today which reads:

Pour me permette de vous identifier auprès du Répertoire National de l’Assurance Maladie avec un numéro définitif d’immatriculation, je vous invite à me retourner (par email ou par courrier uniquement) ce document accompagné des pièces demandées ci-apres en deux exemplaires: Une copie de l’extrait d’acte de naissance original comportant less noms et prènoms de vos parents, plurilingue ou avec la traduction assermentée par un traducteur de la Cour d’Appel en France ou par votre Consulat en France. Une copie d’une pièce d’état civil en cours de validité. La photocopie de la double page du passeport pour les ressortissants de la UE

The birth certificate I own does not have all of the information on it that they want it would seem, as it only says my name and surname, sex, date of birth, place of birth and the registrar’s name and details.

I have emailed RSI to ask if they will accept my marriage certificate alongside this smaller birth certificate, as my marriage certificate has both of my parents names on it.

1. Do you have any experience of this and also of having to have the birth certificate translated?
2. Can you recommend a translator de la Cour d’Appel en France?
