What is declaration commune des revenus des professions independantes revenues 2010?

I have received this form today from CERFA which must be returned by 6/5/2011 or they will add a penalty. I am part of the auto-entrepreneur scheme (recently closed) under the commercial option. I have read it through, understanding little - my earnings are minimal for 2010. I would normally go into a local office but this address is dept 44 and we are in 72 Can anybody shed any light?
