What is the role of RSI in France?

We have only been in France for three months and so the family do not yet have carte vitale. We have been given forms to fill out by the local doctor in order to be treated by him, he said to give them to caisse primaire d’assurance maladie (CPAM) when handing in our carte vitale documents. However as I have just joined régime social des indépendants (RSI) should I give them the doctors forms instead? I am not entirely sure of the role of RSI.
The form from the doctor is called declaration de choix du medecin traitant and my two children and husband have one also.
