When do I need to complete declaration trimestrielle de recettes?

I have just received my first form dated 5th April with a deadline for receipt of 31/7/13. Do I complete this for January to March turnover, or wait and complete it for January to March and April to June together? Do you have either a completed form or a video showing what needs to be completed on the form anywhere?
