When should I pay declaration mensuelle de chiffre d’affaires?

I have received a declaration mensuelle de chiffre d’affaires in July and August but a date limite de reception as 30 November 2015.

Should this be paid monthly or is this different to the monthly payments?
It also states two tax rates, i.e. 13.3% and 22.9%. As a gite am I expected to pay just one of these?
Is income tax then on top of these social charges?
Which one is paid monthly on the auto entrepreneurs website?
