Where do I find an autorisation de prélèvement form?

I completed my first declaration online this week. It went through and I printed out the accusé de réception. My only concern is if the payment will be taken from my bank account. I see that Denver had a similar question and your reply states that you need to have sent a copy of the autorisation de prélèvement to the bank. I don’t remember this form from when I set my account up nor sending the bank any paperwork. Where would you advise where I can find / obtain this form?
