Who do I return the declaration de choix du medicine traitant to?
I called RAM today as I had not received an attestation de couverture sante yet. I already have a carte vitale (with MGEN) but going professional liberales has complicated things for me. RAM stated that they wanted the declaration de choix du medicine traitant form along with my RIB. Also, they said that they do not need to provide attestation de couverture sante for me. So do I need this attestation? What purpose does it fulfil?
Also, I was reading on the forum that the declaration de choix du medicine traitant should be returned to RSI. However, RAM said they wanted this form. So who should I provide the original traitant form to (as I only have one copy)? Or should both bodies have the original form?
Also, RAM requested my RIB, I assume they will need to deduct health charges? How often will this be taken out, starting from when and more importantly how much? I started by business in France early July 2016.
